Background: Detailed knowledge of coracoid process morphology is essential for safe shoulder surgery and preoperative planning. Population-specific morphometric data may improve surgical techniques and implant selection. This study aimed to evaluate the relationship between coracoid process morphometry and other scapular parameters using three-dimensional CT reconstruction and dry scapulae in an Iranian population.
Methods: A total of 96 specimens (29 dry scapulae, 67 CT scans) were evaluated. Coracoid length, coracoacromial distance, scapular width, and scapular length were measured using a digital caliper on dry scapulae and using multiplanar reconstruction and volume-rendering techniques on three-dimensional CT images. Measurements were compared between methods, sex, and side using independent-samples t-tests and the Mann–Whitney U test; correlations between coracoid length and other scapular parameters were assessed using Pearson's correlation coefficient.
Results: No significant differences were observed between dry scapulae and CT-based measurements for coracoacromial distance, scapular width, or scapular length (P > 0.05), while coracoid length showed a borderline difference between the two methods (P = 0.05). In CT measurements, males showed significantly greater scapular width, scapular length, and coracoid length than females (P < 0.05 for all), whereas coracoacromial distance did not differ significantly between sexes (P = 0.27). Coracoid length demonstrated strong positive correlations with scapular length (r = 0.85, P < 0.01) and scapular width (r = 0.71, P < 0.01).
Conclusion: Coracoid process dimensions were positively associated with overall scapular morphology. These findings provide morphometric data that may assist preoperative planning and improve surgical decision-making in shoulder procedures. This study provides reference morphometric data for the coracoid process and scapular parameters in an Iranian population.
